Information Sheet

• I. The Parties: This 360 Photo Booth Agreement is made on _______(today’s date should populate) between Crescent Oak 360, LLC & Client ( POC name should be pulled in from information sheet)
• II. 360 Photo Booth: Crescent Oak 360, LLC agrees to provide 360 photo booth services as outlined in (option should populate from info sheet)
• III. Event Information: (Information should be pulled from the info sheet) Crescent Oaks 360, LLC will arrive 1 hour before the 360-time requested on the information sheet for set up.
• IV. Payment: (Pull in POC name) will pay a total of (pull in an amount from info sheet) for Crescent Oaks 360, LLC services that will be provided on (pull in event info). Payment should be made as follow:
  • a. Deposit: As part of this agreement, $100 shall be paid to secure the booking. The deposit shall be credited and deducted from the balance. Said deposit shall be deemed non-refundable if, for any reason, the client attempts to change or terminate this agreement.
  • b. Balance Due: The balance for the selected option is due 30 days before the event date (populate 30 days before the event here). Once the balance due date has passed, any amount paid, by the deposit or other reasons, by (pull in POC here) will become nonrefundable, and Crescent Oak 360, LLC shall have no obligation to perform services under this agreement.
  • c. Extended hours: An overage in time will be billed at $100/ per hr. Extended hours (s) must be paid before additional hours are provided. If the event ends earlier than expected, no refund will be given.
  • d. Date Changes: Any date change request must be made in writing as soon as possible before the event and is subject to availability.
  • e. Crescent Oak 360, LLC will refund the amount paid if Crescent Oak 360, LLC needs to cancel.
• V. Terms and Conditions: Crescent Oaks 360, LLC and (Pull in POC contact from info sheet)
  • a. Operations: Crescent Oak 360, LLC will set up, manage, and break down the 360-photo booth. Crescent Oaks 360, LLC will have qualified support staff to operate the booth.
  • b. Space and Placement: (Pull in client name from info sheet) will arrange appropriate space and access for the 360-photo booth.
  • c. Liability:
    • i. Crescent Oak 360, LLC is not responsible for any damage caused to the venue by a guest while utilizing services.
    • ii. All users of the 360 platform are participating at their own risk. Crescent Oak 360, LLC will not be held responsible for any incident. The point of contact is responsible for informing guests before the event.
    iii. Inebriated guests will not be allowed to use the 360 platform.
• VI. Governing Law: This agreement shall be construed and governed in accordance with the laws located in the state where the event is taking place.
• VII. Entire Agreement: This agreement constitutes the entire agreement between the Partiers. A modification or amendment of this agreement shall be effective only if in writing and signed by both parties.
• VIII. Execution: Crescent Oak 360, LLC and (pull in client’s name from info sheet) each represent and warrant to the other that each person executing this agreement on behalf of each party is duly authorized to execute and deliver this agreement on behalf of that party.
